Police Check - Required For All Volunteers

This form must be delivered to and picked up from the police station by the applicant. HMH cannot do this for you. All coaches, managers and team personnel MUST have a valid police check on file with HMH that is less than 2 years old. Without a police check on file, YOU WILL NOT be permitted on the ice or in the dressing rooms.
